A Study on the Problems of Diagnosis and Management of Mediastinal Tumors


Abstract
Author studied 20 cases of mediastinal tumors on the problems in diagnosis and management of the tumors, and the results were as followings.
1. The natures of each tumor of mediastinum could be generally obtainable by postero-anterior and lateral views of chest x-ray, and some atypical locations were observed among the malignant lesions. The more details of diagnosis in determination of malignancy were needed bronchography, esophagography and mediastinoscopy.
2. Malignant changes were seen in 2 cases out of 10 cases of benign mediastinal tumors.
3. Possibilities of malignancy were likely recognized when the margin of tumor shadow was irregular and bilaterally expanded in the mediastinum on postero-anterior views.
4. The transitional angle between opaque shadow of the tumor and mediastinal shadow was usually obtuse in the tumors of anterior mediastinum, but the lung tumors revealed usually acute angle.
5. The post-operative prognoses were very poor in malignant mediastinal tumors, and it seemed to be very important to determine pre-operatively the malignancy of the tumors.
